A NEW program is offering short term support for elderly patients looking to return home and regain independence after leaving hospital.
The Millicent and District Hospital and Health Service recently introduced the Transition Care program and has since helped four patients get back on their feet.
Among those is Nan Masters who despite only using the program for two days, said it was good for people with “nowhere to go in between”.
It also gave her the opportunity to return home to Millicent as opposed to staying in Mount Gambier where she was treated.
Millicent Hospital Evening Auxiliary group generously offered $7000 to set up a room in the hospital, which would be used to run Transition to Care.
The funding covered associated costs of furniture and new blinds, which made the room “feel more like home” for patients in the program.
President June McIntyre said the fundraising committee was pleased to support the Millicent and District Hospital as its services are vital to the community.
“The hospital is at the heart of our community and we are always happy to help any way we can,” she said.
“We know this program will really benefit older patients who need extra support to feel confident as they leave hospital and return home.”
Ms McIntyre added with such a small group on board the committee, they were “desperately” seeking new members.
The program targets elderly patients who have had acute illness or injury and need further help and time to complete their recovery.
It aims to give patients the confidence to return home independently and includes therapy, nursing support and personal care.
Transition Care is jointly funded by SA Health and the Federal Government.
